Personal Health Coaching from Baylor Scott & White serves as an extension of the care from your primary care provider. For those times when managing diabetes feels challenging, you’ll get the support you need with a real-life coach and personalized program, accessed through your MyBSWHealth app.
The program is free of charge to you.
If you are living with Type 2 diabetes, the Personal Health Coaching program connects you with resources between visits with your provider.
The program includes:
- A coach with experience in helping people living with diabetes. Your coach will be integrated into your healthcare team and can suggest lifestyle changes that work with your existing treatments.
- Individualized and confidential coaching sessions over 12 weeks.
- Communication with your coach when you need help or advice.
- The ability to self-schedule appointments with your coach.
- Educational resources and tips from your coach, all within the MyBSWHealth app.
- The ability to sync your blood sugar monitor and digital scale data, accessed by your coach, so they can provide feedback and guidance based on your health data.

MyBSWHealth app
You can set up your first Personal Health Coaching appointment in the MyBSWHealth app.
Creating a MyBSWHealth account gives you access to communicate with your health coach, manage your appointments and use our many other virtual care services.
How personal health coaching works
Once you’ve signed up, Personal Health Coaching is available through the MyBSWHealth app. You can schedule time with a coach that fits your schedule. When you have your first meeting, your coach will spend time getting to know you and talking with you about your goals and lifestyle. You’ll work together to create a personalized plan.
Your provider will stay connected and get updates from your coaching sessions. At the end of your program, your primary care doctor will receive a summary from your coach of your progress.

Virtual coaching sessions on your schedule
Over the course of 12 weeks, you and your coach will meet for 30-minute, personalized virtual coaching sessions by phone or video. In addition to virtual sessions, your coach will send you messages through the MyBSWHealth app so you can stay connected and adjust any sessions based on your schedule.

Get support and guidance through secure messaging
Your coach will connect with you through the MyBSWHealth app on the weeks that you don’t have a virtual coaching session to recap the goals that you have set together, as well as provide information and education.
You can also communicate directly with your coach via secure messaging in the MyBSWHealth app.

Goals created by and for you
You and your coach will set goals that can help you to better manage diabetes and improve your overall health based on your daily habits and routines.

Continued support after the program ends
Support doesn’t end once you complete the program after 12 weeks. You can opt into an additional 12-week maintenance period with your coach and you can re-enroll in the program after a year if needed.
