Baylor Scott & White HouseCalls program has been caring for the senior adults of North Texas for more than 20 years. Beginning in 1998, Baylor Scott & White HouseCalls has been proud to provide primary care to older adults who are homebound and unable to access healthcare through regular office visits with a physician.
Our goal is to bring quality care to the adult population in the comfort of their home, group home, assisted living facility or special care unit. We take a multi-disciplinary approach to home care treatment that may include physicians, nurse practitioners, social workers and chaplains. Our goal is to ensure that our patients maintain a quality of life and avoid visits to the emergency room or hospital.
The experienced staff of nurse practitioners, physicians assistants and physicians here specializes in geriatric and adult medicine, and they visit close to 1,000 patients who are enrolled in our program. Baylor Scott & White HouseCalls has enjoyed tremendous success in preventing emergency room visits and hospital admissions for our patients. Results show that our efforts have significantly improved the quality of care for our senior patients.

Comprehensive geriatric assessment
The geriatric assessment is a consultation with our patients that enables us to gather important information on the medical, psychosocial and functional capabilities and limitations of our elderly patients. We use this information to develop treatment and long-term follow-up plans; arrange for your primary care and rehabilitative services; organize and determine possible long-term care requirements and optimal placement; and make the best use of healthcare resources.
The geriatric assessment differs from a standard medical evaluation in three general ways:
- It focuses on elderly individuals with complex problems
- It emphasizes functional status and quality of life
- It frequently takes advantage of an interdisciplinary team of providers
Home care coordination
The nurse practitioners and physicians here take a multidisciplinary approach to the care of patients. An important part of our service is to manage our patient's medical and psychosocial needs. We proactively manage the healthcare of our patients in all settings and severity of illness and make every effort to match appropriate resources to a patient’s condition.
Management of common chronic conditions of the elderly
All too often, because there are so many chronic conditions that seem to afflict older persons, there is the mistaken perception that diabetes, arthritis and the like are just “part of growing old”—and nothing can be done about them. The truth is most of these diseases and conditions are treatable and should be addressed by a physician. We help manage most common chronic diseases afflicting the elderly which include:

Pay by mail
To ensure that your payment is correctly applied to your account, detach the slip from your Baylor Scott & White billing statement and return the slip with your payment. If paying by check or money order, include your account number on the check or money order.
Please mail the payment to the address listed on your statement.

Payments to HTPN can be made over the phone with our automated phone payment system 24 hours a day, seven days a week. All payments made via the automated phone payment system will post the next business day. Please call 1.866.377.1650.
If you need to speak to someone about a bill from a Baylor Scott & White Hospital, our Customer Service department is available to take payments over the phone from Monday through Friday from 8:00 AM - 5:00 PM and can be reached at 1.800.994.0371.
