Overview

Our health coverage runs from Jan. 1 - Dec. 31 of each year. Employees, spouses and children who meet the eligibility requirements below are eligible for coverage by our benefits program.

For new hires or newly benefits-eligible employees, you have 30 days from your date of hire or benefit eligibility date to complete your enrollment.

Once completed, your coverage begins right away. If you do not complete your enrollment within this timeframe, you will not be able to elect benefits until the next scheduled annual enrollment period or within 30 days of a qualifying life event.

Medicaid

Do you or a family member in your household have Medicaid coverage?

You could qualify for the TX HIPP program, which reimburses your BSWHP insurance premiums and allows Medicaid to stay in place for you and eligible family members.

Eligible

The following are eligible to enroll:

  • Full-time employee
  • Part-time employee working 20-29 hours per week
  • Spouse or common-law spouse
  • Your children or spouse's children up to the age of 26*, including:
    • Natural children
    • Step children
    • Legally adopted children
    • Children in the process of legal adoption
    • Foster children
    • Children covered by a Qualified Medical Child Support Order
    • Children of whom you have legal guardianship

Note: If you and a dependent both work for Baylor Scott & White, only one can be covered as a dependent.

*Subject to approval, the age limit is waived if the child is unmarried, physically and mentally incapacitated and unable to earn an independent living; dependent on you for at least 50% of financial support; claimed by you as a dependent for federal taxes; and disabled and covered under the plan before age 26 (unless you are a new hire or newly benefit-eligible).

Overview

Important! When you add new dependents on our benefit plans (during initial or annual enrollment, or following a qualified life event), you will receive a request from the Dependent Verification Center. This step is important to ensure your dependents remain covered and only eligible dependents receive benefits.

Documentation

Documentation such as marriage certificates, birth certificates, legal guardianship paperwork and/or tax forms will be required. It is important to submit all requested documentation in a timely manner to ensure eligible dependents remain covered.

CHIPRA Notice

If you or your children are eligible for Medicaid or CHIP—the Children’s Health Insurance Program—and you’re eligible for health coverage from your employer, your state may have a
premium assistance program that can help pay for coverage using funds from their Medicaid or CHIP programs. If you or your children aren’t eligible for Medicaid or CHIP, you won’t be eligible for these premium assistance programs but you may be able to buy individual insurance coverage through the Health Insurance Marketplace.

Consolidated Frozen Pension Notice

BSWH is required to provide this notice to active and terminated employees with a vested benefit in the All Saints and/or Brenham frozen pension plans when a private letter ruling is submitting to the Internal Revenue Service (IRS) requesting church plan status.

Creditable Coverage Disclosure Notice

This notice has information about your current prescription drug coverage with Baylor Scott & White and about your options under Medicare’s prescription drug coverage. This information can help you decide whether or not you want to join a Medicare drug plan.

HIPAA Notice of Privacy Practices

This Notice of Privacy Practices describes how the Baylor Scott & White Health Medical, Dental, Vision, Healthcare Flexible Spending Account (FSA) and Employee Assistance Program (EAP) may use and disclose your protected health information. This Notice also sets out the Plans’ legal obligations concerning your protected health information and describes your rights to access and control your protected health information.

HIPAA Special Enrollment Rights

This notice includes details about if you are declining enrollment for yourself or your dependents (including your spouse) because of other health insurance or group health plan coverage, you may be able to later enroll yourself and your dependents in this plan if you or your dependents lose eligibility for that other coverage (or if the employer stops contributing towards your or your dependents’ other coverage).

Marketplace Notice

Since 2013, the Health Insurance Marketplace—healthcare.gov—has given people a new way to buy health insurance. This notice provides some basic information about the Marketplace and the employer-sponsored medical coverage offered by Baylor Scott & White Health. 

Retirement Plan Notice to Interested Parties

The Interested Party Notice is a document notifying employees, participants and beneficiaries that an application for determination is being submitted to the IRS. The notice also informs these individuals of their right to comment on the plan.

Summary Annual Reports (SAR)

A summary annual report (SAR) summarizes the information that appears in a benefit plans Form 5500. SARs are posted each year for 401(k), 403(b), and Health & Welfare plans.

Summary of Benefits and Coverage (SBC) is an easy-to-understand summary about each medical plan option offered by Baylor Scott & White Health. This summary makes it easy for you to compare coverage between all three network Tiers and how different services and procedures are covered.

This booklet contains Summary Plan Descriptions (SPDs) that describe how the Baylor Scott & White Health employee benefit plans (the plans) work. Each section has important details about specific plans. This section provides important information on eligibility, enrollment and paying for coverage.

Women’s Health and Cancer Rights Act

This notice provides details about certain benefits under the Women’s Health and Cancer Rights Act of 1998 (WHCRA) if you have had or are going to have a mastectomy.
