The Baylor Scott & White NCQA-certified coaching program provides employees with four free sessions and direct access to trained and experienced Wellness Advisors. With wellness coaching, participants gain guidance in the areas of nutrition, exercise, stress-management and behavioral changes to achieve optimal well-being of body, mind, and spirit.

    That means our program meets NCQA criteria, the structure and processes we have in place are evidence-based and have been proven to support your health and improve outcomes!

    Care management

    Your Baylor Scott & White Health (BSWH) doctor works alongside a dedicated care management team to help you get—and stay—healthy.

    You will receive a call from us. Please answer the phone or check your voicemail if we missed you. Our care managers offer personalized assistance with the following:

    • Healthcare needs: Your care team of nurses, social workers, health coordinators and pharmacists help you get the care you need when you need it. 

    • Scheduling appointments: Get help scheduling appointments.

    • One-on-one guidance: A nurse care manager will call you in between visits to your doctor or after a hospital stay to make sure you understand your care instructions and educate you on medications you might be taking.
